I paid out of pocket for my treatment. How do I make sure the claim payment comes to me instead of to my dentist?

On the claim form, there are two boxes for "Authorizations." Box 36 is always signed because it indicates that the patient is responsible for paying the costs of the treatment if the insurance plan does not cover all or a portion of the claim.

Box 37 authorizes the insurance carrier to direct payment to the dentist instead of to the member. If you want the payment to be sent directly to you, the member, then simply refrain from signing box 36!


It's a good idea to double check with your provider to make sure they don't automatically fill this box with "Signature on File" and the date.
