How do I register for the Provider Portal?

Start here:

First, select “Create Account."


Then, fill out the form with your information and hit "Submit!"


If your practice information is already in our system, you'll have immediate access to the Provider Portal! If you're not already listed, we need to verify your credentials and create your account on our end, so it'll take about a day to finish the registration process. Once you're in, you'll receive an email from us!
